At the end of this questionnaire are options for how to submit your electronic deposit. Remember, your deposit is to indicate serious interest in partnership, and will be fully refundable until we begin construction or if for any reason either of us decide not to move forward with partnership!

The total number of therapeutic professionals we can comfortably accommodate is limited so we ask for a $100 refundable deposit with each therapeutic application to ensure applicant interest.
